As the requests for Dr. Robert L. Evan’s availability have increased, we want to ensure we get back to you in a timely manner. In order to do so, please submit this request ACCURATELY, double-check the information you submit as this may severely impede our ability to contact you in a timely manner. Completing this form allows us to verify your eligibility for benefits (which we require prior to scheduling) and it allows us to provide you with Dr. Evan’s availability and add you to the schedule if there’s an opening. Please allow 72 hours for a response by email, checking it frequently as the appointments are honored on a first come first serve system. If there is no availability, we will notify you by email as well and will place you on a waiting list if you desire.
You understand that completing this form does not guarantee services. This is the new protocol as this is the most efficient method at this time. Calling the office and leaving a message may lengthen the response time.
